As of 4/1/2017, first time clients/patients MUST secure your appointment with payment of an appointment deposit or an Appointment Guarantee Fee, of one hundred & sixty dollars [$160]. This fee GUARANTEES your appointment day, time & date where others cannot fill it or take it.
The appointment deposit will serve as a credit or, in essence an "advance payment" for ANY or ALL expenditures you would ensue like ancillary services or materials that your program calls for. For example: completion of FMLA/Disability Forms, Books, CD's, etc.
This amount can or will cover the "No Show/No Postponement/ No Cancellation Fee" if notice 48 hours prior to your appointment isn't given to me by you. This is a "STAND ALONE" POLICY THAT IS THE $160 WILL BE CHARGED & COLLECTED IF THIS POLICY IS VIOLATED THROUGHOUT ANY DEGREE OR LONGEVITY OF THERAPY - BE CAREFUL!
This appointment is confirmed by submitting the (Debit or Credit) Card number, Expiration Date & Security Code (typically a three digit number on the back of the card) via the encrypted
appointment website TherapyAppointment.com.
Fully register & pick out your appointment day, date & time first. I will then send you a Welcome E-mail via the encrypted e-mail system at www.TherapyAppointment.com you can then respond with the above information back to me or you can simply & quickly NOW take care of this below , , ,
